At Compass, our goal as the Health Information Management department is to ensure the medical record is complete and made accessible in a timely manner while protecting patient(s)' rights and confidentiality.
Process for Requesting Medical Records
To request medical records be released to yourself and/or an appointed representative, please print, complete and sign the form below.
Return to Compass Health Center via fax to: 773-409-8592 or via email to healthrecords@compasshealthcenter.net.
Please click the button below for a copy of the authorization that needs to be completed with instructions on how to properly complete it.
Please allow up to 14 business days after your written request has been received for records to be sent.
Should additional information be needed to process your request, a representative will reach out.
