You have the right to your medical record.
At Compass, our goal as the Health Information Management department is to ensure the medical record is complete and made accessible in a timely manner while protecting patient(s)’ rights and confidentiality.
Process for Requesting Medical Records
To request medical records be released to yourself and/or an appointed representative, please print, complete and sign the form below.
Return to Compass Health Center via fax to: 773-409-8592 or via email to healthrecords@compasshealthcenter.net.

- Please allow up to 14 business days after your written request has been received for records to be sent.
